International Congress on Ambulatory Surgery. The Congress will take

All Fellows are encouraged to complete the survey either electronically
or by hard copy, but not both. Your personal unique identifier must be
entered into the fields and has been sent to your preferred email or
postal address.
The first question requires you to include this unique identification
number so that we can cross off those who have completed the census. The
unique ID number is specific to you for the purposes of this census.
Only the College's Workforce Planning & Research Officer will have
access to your identity via the ID number. In all other respects your
responses will be anonymous.

The Children's Surgical Centre (CSC) has provided rehabilitative surgery
for poor rural Cambodians since 1998. In this time patient numbers have
grown exponentially, entirely through word-of-mouth advertising, with a
