Fellowships  of the RACS are awarded in nine surgical Divisions – Cardiothoracic Surgery, General Surgery, Neurosurgery, Orthopaedic Surgery, Otolaryngology Head and Neck Surgery, Paediatric Surgery, Plastic and Reconstructive Surgery, Urology and Vascular Surgery.  Each Division has a Board for training in that specialty. The Boards can appoint regional subcommittees to advise on training facilities in the regions, and to be available to advise trainees in the specialty. There are also Fellows in Ophthalmology, and Obstetrics and Gynaecology.

Additionally, the College is an umbrella organisation representing the interests of all Fellows in whatever specialty they practice. The following Societies and Associations include Fellows who participate in the activities and educational programs of the College.
