Please note, from 2009 the dates Trainees must submit assessment forms have changed. All Vascular SET trainees must submit their Assessment form three times a year i.e. at the end of May, September and January. Download the Trainee Assessment Form (PDF).

Mini-Cex and DOPS Forms

The Mini-Cex and DOPS forms must be submitted three times a year, i.e. at the end of May, September and January.
