SET1 forms for trainees starting in 2012

Download the files below as required. Trainees and Surgical Supervisors will be advised when there are form updates.

Trainees are advised, in accordance with the Training Program Regulations, that the above forms are compulsory for all trainees to complete and must be submitted to the Executive Officer, Board of Paediatric Surgery by the communicated due dates after each 3 month rotation. Failure to submit documentation by the due dates will result in the Board being unable to assess the training period.
