BQA online portal
Through this portal, surgeons can review their practice, see their reports, download their data and compare their results with predetermined quality thresholds (benchmarks) for several key performance indicators. Help and support is also available via the portal.
How to submit
Data is submitted directly through the online portal at the link above.
Surgeons wishing to have a proxy enter data for them can request Data Manager access for that person by submitting the Data Manager access form (PDF 907.73KB). This Data Manager access can be amended at any time by submitting the amendment form (PDF 433.62KB).
Alternatively, surgeons entering similar data into an existing database, such as a hospital database, register or local audit, may be eligible for the institutional upload program. The existing database will need to have a significant number of annual cases and have sufficient commonality of fields with the BQA. Contact the audit helpdesk to discuss the eligibility of your database.
BreastSurgANZ expect all cases to be submitted by April 30 of the year following diagnosis. Participation certificates will be issued shortly after this date as an acknowledgement of contribution.
Data entered after 30 April may not be taken into consideration at the time of calculating annual participation. If there are extenuating circumstances, contact BreastSurgANZ regarding an extension at the link below.
NOTE: The deadline for case submission in 2020 has been extended by one month. All 2019 cases are to be submitted by 1 June 2020.
Data collection forms
Participants who prefer to collect data onto paper forms and then transcribe that information into the system are welcome to use the BQA Data Collection forms.
The full dataset form includes all fields collected by the BQA, both mandatory and optional, thus allowing the full research and record keeping potential of the audit to be utilised.
Minimum dataset (MDS) forms provide a shorter alternative to the full dataset while including all data items necessary for threshold calculations on key performance indicators.
Patient information and opt-out consent forms
All patients need to be informed of the audit prior to having their data entered, giving them the opportunity of opting-out of having their medical information recorded.
The patient information sheet (PDF 130.29KB) has been designed for this purpose.
