BQA online portal

BQA online portal

Through this portal, surgeons can contribute to the BQA, review their practice and compare their results with predetermined quality thresholds (benchmarks) for several key performance indicators.

How to submit

Data is submitted directly through the online portal at the link above.

Surgeons wishing to have a proxy enter data for them can request Data Manager access for that person by submitting the Data Manager access form (PDF 289.53KB). This Data Manager access can be amended at any time by submitting the amendment form (PDF 434.4KB).

Alternatively, surgeons entering similar data into an existing database, such as a hospital database, register or local audit, may be eligible for the institutional upload program. The existing database will need to have a significant number of annual cases and have sufficient commonality of fields with the BQA. Contact the audit helpdesk to discuss the eligibility of your database.

Submission deadline

BreastSurgANZ expect all cases to be submitted by 30 April of the year following diagnosis.

Data entered after 30 April may not be taken into consideration at the time of calculating annual participation. If there are extenuating circumstances, contact BreastSurgANZ regarding an extension at the link below.

Data collection forms

The BQA provides the below forms for personal use if you wish to collect data on paper prior to entering through the online interface. This can be useful for surgeons who want to write the necessary information down and have someone else enter for them at a later time. We do not accept the paper forms as submission to the BQA.

Invasive cases with neoadjuvant treatment (PDF 245.54KB)

Invasive cases with no neoadjuvant treatment (PDF 231.49KB)

DCIS cases (PDF 188.64KB)

LCIS cases (PDF 187.6KB)

Follow Up appointment (PDF 159.56KB)

Patient information and opt-out consent forms

All patients need to be informed of the audit prior to having their data entered, giving them the opportunity of opting-out of having their medical information recorded.

The patient information sheet (PDF 135.1KB) (PDF 358.42KB) has been designed for this purpose.
